Can the insurance company deny this claim?

i have asthma. i was out of my meds, had an asthma attack and went to the ER ( not the first time this happened). they took a chest x-ray which was normal then the doctor insisted i have a CT scan to rule out a pulmonary embolism even though i had a normal d-dimer. the insurance company denied payment for the CT
